The Intake Coordinator is responsible for providing a professional level of administrative and client-facing support to assist departments of Foothill Unity Center, Inc. Description:
Case Workers at Foothill Unity Center serve as comprehensive resource navigators who provide holistic support to individuals and families facing poverty, instability, and systemic barriers. They are trained to assist clients across a range of essential service areas—including housing, health, finance, and job development—and are equipped to assess needs, build action plans, and connect clients to critical resources.
Each Case Worker may develop specialized expertise in one or more of these service areas depending on their interests and the needs of the organization. Regardless of specialty, all Case Workers are cross-trained to provide integrated support tailored to client goals and circumstances.
Essential Duties and Responsibilities:
• Conduct holistic intakes and assessments to identify client goals, barriers, and service needs across housing, health, finance, and employment.
• Develop individualized service plans focused on building stability and promoting self-sufficiency.
• Provide hands-on budgeting support and financial coaching to help clients manage income, expenses, and debt.
• Assist clients with eviction prevention, utility assistance, rental aid, and emergency resources.
• Support clients in accessing healthcare, mental health resources, and public benefits.
• Offer guidance and resources related to workforce readiness, training programs, and job placement.
• Advocate on behalf of clients with landlords, employers, utility providers, and service agencies.
• Collaborate with external partners, including legal aid, housing providers, workforce agencies, and healthcare institutions.
• Accurately track and document client interactions, progress, and outcomes in CSMS and HMIS, ensuring confidentiality and HIPAA compliance.
• Provide follow-up services to ensure long-term client success and stability.
• Participate in community partnerships, coalitions, and outreach efforts to improve services and expand resource access.
