CASE MANAGEMENT PROGRAM

The driving strategy and standard of care practice that supports our core programs through assessment, case planning, care coordination, advocacy, resource linkages, and follow up. Individualized client plans allow our case worker team track progress and make adjustments specific to personal situations. Case management provides access to a complete continuum of services, a safety net that improves the odds of success for an individual to move from poverty to economic security.

CASE MANAGEMENT SERVICES

We refer clients to a network of more than 100 organizations that can help them return to stability via employment counseling, shelter, health care, family support, mental health services, eviction prevention/move-in assistance and advocacy.

Are you a single parent who has custody of a child under the age of 18?

If you are, you may be eligible for help from the California Department of Child Support Services with obtaining child support from the father or mother of your child.

What types of services would the California Department of Child Support Services provide?

Some of the services the California Department of Child Support Services provides to eligible parents include:

  • Locating a parent
  • Arranging for paternity testing
  • Establishing a support order
  • Enforcing a support order

We collaborate with Santa Anita Family Service to deliver this assistance.

Clients can enroll in Food Stamps and Medi-Cal programs onsite.

We provide bus tokens and taxi coupons in collaboration with the International Institute of Los Angeles.

The program aims to reduce the barriers to employment and help the whole family, thereby ending the cycle of poverty for the next generation. Services include on-site work experience training with job readiness preparation, academic and vocational counseling, resume assistance, certifications, mentoring, financial literacy workshops, job search and assistance with employment placement, employer relations and development, and referrals to community agencies for training.

 We provide short-term case management focused on homelessness prevention. Services may include rental and utility assistance, along with referrals to trusted community partners for longer-term housing support.

All individuals and families are assessed to determine eligibility, and services are provided based on specific program criteria and available resources.
