Job Title: Health Services Supervisor
Department: Health Services Department

Summary:
The Health Services Supervisor plans and directs the goals of the health services department, implements procedures, assures services are rendered efficiently, and case management to clients of the organization. Responsibilities include case management, staff management, coordination, and development of health services programs. The ideal candidate will demonstrate effective time management, commitment to quality and accuracy, possess a high level of emotional and professional intelligence, and is accountable for all deliverables, communications, and performance. It is preferred that the ideal candidate have in-depth knowledge of the San Gabriel Valley community or comparable diverse communities.
Essential Duties and Responsibilities:
Case Management
- Oversee case management program to assess program needs, best practices, and ensure funding compliance.
- Oversee reporting of abuse, neglect, or protective services to vulnerable populations.
- Manage all existing and new Health Services programs; maintain accurate records and collaborate in preparation of reports, as required by funders, in an accurate and timely manner.
- Execute Memorandum of Understanding with community partners and stakeholders.
- Identify and assess potential needed services for program implementation.
- Establish protocols, rules, and client intake/screening procedures in accordance with program and budget needs.
- Establish community partnerships that leverage resources or that lead to funding opportunities.
- Conduct assessments internally and externally to improve client services programs and objectives.
- Support the Director of Programs and direct staff in establishing, updating, and overseeing departmental policies and procedures, annual budgets, program objectives, and other tasks as required.
Staff Management
- Oversee members of the Health Services team including but not limited to case managers, support staff, interns, and trainees.
- Meet regularly with staff for direct reports; supports professional development and program tasks.
- Organize and coordinate team meetings, both ongoing and ad-hoc groups relating to projects.
- Ensure that Health Services staff adheres to the organization’s policies and procedures; conduct employee-performance reviews in accordance to agency guidelines.
- Provide case managers direction, resources, and referrals to support work tasks.
- Communicate emerging issues in a timely manner to Director of Programs.
- Coordinate with Social Services Department to ensure whole-person care.
- Demonstrates an understanding of basic HR guidelines and practices, including employee relations, performance management, and compliance with organizational policies and employment laws.
Community Partnerships
- Assist Director of Programs in establishing relationships with other agencies, school, monitors, and service providers.
- Participate and attend community events and external meetings to represent the organization.
- Develop and maintain positive working relationships with health and social service providers and assure that staff have accurate and up to date information about access and referral to these services.
- Other duties as assigned.
Supervisory:
This position oversees members of the Health Services team including but not limited to case managers, support staff, volunteers, interns, and trainees

Education/Experience
Required education and experience. An equivalent combination of education and experience is acceptable.
- A Bachelor’s degree in a Human Services or related field, or 3-5 years of case management and supervisory role that provides the equivalent knowledge, skills, and abilities may be substituted for a required education.
- LVN, RN, or other accredited nursing certification are required and may substitute for a bachelor’s degree based on relevant experience.
- Experience with support of multiple and diverse programs and/or operations.
- Experience using a client information database for grant compliance and management reporting.
- Experience working in a high-paced, demanding environment.
- Familiarity with quality assurance practices and reporting to public and private funders.

Job Title: Volunteer Coordinator
Summary
The Volunteer Coordinator will coordinate the resources of the Volunteer Department in the delivery of FUCI’s programs and services. This includes overseeing volunteers and providing staff who supervise volunteers trainees with training, direction, resources, and tools. The ideal candidate for this position will exhibit effective time management, dedication to equitable service delivery, a high degree of emotional intelligence, and accountability for all deliverables, communications, and performance.
Essential Duties and Responsibilities
- Plans and coordinates the volunteer programs/services and events; including, but not limited to volunteer appreciation events quarterly and annually.
- Assesses the need for volunteers, and the number needed for each event, to enhance program/service delivery.
- Develops and implements effective strategies to recruit the right volunteers with the right skills, and to work with the right staff employee; Identifies volunteer assignments that provide meaningful work for volunteers.
- Develops and implements an intake and interview protocol for potential volunteers to ensure the best match between the skills, qualifications, and interests of the volunteers and the needs of the organization.
- Recruit and train community members, college students or recent graduates, and all volunteers who support the infrastructure of programs.
- Develops and implements goals and objectives for the volunteer program which reflect the mission of the organization.
- Maintains a database of volunteer information and skills, matching volunteers to opportunities that suit their skills.
- Maintains all volunteer reporting, coordinating the tracking of and reporting on all volunteer hours and activities.
- Ensures that the mission and values of the organization are upheld and that the face of FUCI reflects compassion for all, integrity in practices, and professionalism overall.

- Work with the Marketing Coordinator to develop marketing and outreach strategies to promote our organization, Volunteer Program, and Volunteer Opportunities to businesses, organizations, and the community at large.
